Information for Instructors

Providers/Instructors seeking approval from AMTA are required to complete and submit this AMTA Continuing Education Application Form along with supporting documentation, to the Education Chair, Ruth Anselm at 646-250-2121, email; education@amta-ny.org.

If you are already an approved instructor with the AMTA chapter and would like to apply to teach another class, please fill out this AMTA Continuing Education Application form for an approved instructor requesting to add a class and submit to the Education Chair, Ruth Anselm at 646-250-2121, email; education@amta-ny.org.

Applicants will be required to have a personal interview to discuss their qualifications and course content.
